Preclinical Bottlenecks

Gene Complexity: The large NF1 gene causes a high rate of spontaneous mutations and severely complicates targeted gene therapies.
Tumor Heterogeneity: Plexiform neurofibromas (PNs) and malignant peripheral nerve sheath tumors (MPNSTs) display highly variable responses to standard therapies.

Solution: Zebrafish NF1 mutants were created to mirror human pathophysiology. The models were stable double mutants, which exhibit skeletal deformities that progress through pain, inflammation and neurofibromas leading to learning difficulties. In late stage they progress to severe skeletal deformities.

Clinical & Diagnostic Challenges

Phenotypic Variability: Symptoms range from mild skin manifestations (café-au-lait spots) to severe orthopedic complications and aggressive sarcomas.
Malignant Transformation: It is clinically difficult to predict and detect when benign PNs will transform into aggressive, life-threatening MPNSTs.
Drug Resistance: Current MEK inhibitors (such as selumetinib) show limited efficacy against high-grade gliomas and adult PNs, highlighting an urgent need for alternative combination therapies.

Solution: A complete pathway map of NF1 zebrafish vs human NF1 pathology was identified. NF1 zebrafish show higher levels of CTGF, Il 8 and MMP 9 whereas the Wild Type show lesser levels of neuronal, inflammatory and MAPK signalling pathways.

Target Discovery & Translational Hurdles

Surrogate Endpoints: Identifying reliable biomarkers to measure early drug efficacy in clinical trials is challenging due to the disease's unpredictable progression.
Approved MEK Inhibitors & Label Expansion for comparator reference include Mirdametinib (Gomekli™), Selumetinib (Koselugo), PAS-004 and in trials Luvometinib (FCN-159 / Tunlametini. Identifying a relevant comparator based on the test compounds mechanism of action is of prime importance taking into account the option of Non-MEK Targeted Therapies .
